The compliant is to be made in the written form and generally should include the following information:

- name of the court to which the compliant is filed;

- names of the parties including their address of location or residence and their registration numbers (or the tax numbers if the party is the natural person);

- amount of claim (if the same is subject to the money evaluation);

- statements of the fact on the grounds of which the plaintiff argues his claim;

- statements of the evidences, which the plaintiff intends to appeal to in order to support the claim;

- reference to appropriate procedural and material legislative provisions, which support the claim;

- the remedy sought by the plaintiff;

- other information, which is deemed important and relevant by the plaintiff;

- information about use of the preventive measures;

- the list of the documents attached to the claim.

Documents attached to the compliant should include proof of payment of the court fees; representatives are to submit valid powers of attorney (a good copy of the same). Usually, the attached documents also include copies of the written evidence in the plaintiff's possession at the moment of filing the claim.

Plaintiffs in civil and administrative proceedings have to submit the court with additional copies of the compliant and attachments for each party involved in the court proceeding.

Plaintiffs in commercial proceedings have to send copies of the compliant and attachments to the defendant and third parties (if any) prior to filing the papers to the court and provide the court with valid proof of such sending.
